What should I do with my Medical Clearance form?
Please return the Medical Clearance as soon as possible to us via fax at 301-494-3333. Please also bring a copy with you on the day of your surgery.

Are there any dietary restrictions before my procedure?
You must fast (do not eat, drink or chew on gum) after midnight of the night before your procedure

What should I bring?
Please bring:

All of the patient's medications, including inhalers and nebulizers!

If requested by our staff, Medical Clearance.

Patient's medical and dental insurance cards.

Accompanying adults should bring photo ID.

Non-biological parents and guardians must bring proof of guardianship or notarized power of attorney.

Leave all necklaces, rings, earrings, watches, other jewelry and valuables at home.

Bring an extra change of clothes. For younger patients, please bring extra pull-ups or diapers and a stroller to transport them from the facility to the car after their procedures.

Anything else requested by our staff.

What should I wear?
Please wear comfortable, loose fit clothing.
Remove all nail polish. Nail polish interferes with the sensors that monitor oxygen levels.

What if I get sick?
Please call and report any change in medical status, however minor, on the day of procedure, including but not limited to, cold, flu, fever, suspected pregnancy, newly discovered allergies, or changes to medications.
